What Youu2019ll Do As a Licensed Master Social Worker at Eddy SeniorCare PACE, you will: + Provide Comprehensive Social Work Services + Conduct psychosocial assessments for participants and families. + Evaluate home environments and determine support needs. + Deliver ongoing counseling, education, and crisis intervention. + Support participants adjusting to illness, disability, or changing independence. Coordinate Across the Continuum of Care + Participate in interdisciplinary care team meetings. + Integrate your social work treatment plan into the participantu2019s overall Plan of Care. + Manage transitions between home, ED/hospital, subacute rehab, respite, and more. + Link participants with essential community resources and benefits.
