Job Description

Summary This position is responsible for the delivery of social work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed. The Social Worker is responsible for communicating with health care team and documenting all care activities. Essential Responsibilities/Accountabilities u2022 Interprets to the home health team the social factors in the patient situation that may have bearing on his/her progress. u2022 Helps patient and family understand and accept the illness and to anticipate its accompanying adjustments. u2022 Coordinates resources to alleviate the financial impact of illness and assists patient and family in planning for payment of medical care. u2022 Determines the need for continued social work service and draws upon other community agencies that can further assist the patient and family. u2022 If necessary, assists with alternative planning for patients using appropriate community resources. u2022 Assist with discharge planning from Lifetime Care program(s). u2022 Serves as liaison with social work departments and other health and social agencies in the community. u2022 Keeps social work staff informed of community and agency issues. u2022 Consistently demonstrates high standards of integrity by supporting the Lifetime Healthcare Companiesu2019 mission and values and adhering to the Corporate Code of Conduct. u2022 Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures. u2022 Regular and reliable attendance is expected and required. u2022 Performs other functions as assigned by management. Minimum Qualifications u2022 Bacheloru2019s Degree in health related field with a minimum two years related experience. u2022 Home visiting experience strongly preferred. u2022 Proven psycho social assessment skills. u2022 Effective verbal and written communication skills essential. u2022 Ability to stay organized and flexible important. Possession of a valid NYS driveru2019s license and own car required. u2022 Requires pre-employment physical exam as required by NYS Dept. of Health. u2022 Demonstrates strong communication (both written and verbal), interpersonal, organizational, and time management skills. u2022 Basic computer skills are necessary. Experience with documentation in an electronic medical record u2022 With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary. u2022 Demonstrated ability to effectively manage time and caseload autonomously. u2022 On Call may be required. Level II u2013 require similar qualifications as level I, as well as: u2022 Bacheloru2019s Degree in Social Work with a minimum of five years direct Social Work experience. EDUCATION: LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. PAY RANGE: $60,500.00 - $73,000.00 CITY: Newark POSTAL CODE: 14513 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
