Job Description

We are seeking a qualified individual with a New York State License to practice as a Registered Professional Nurse, along with certification by the New York State Education Department as a Nurse Practitioner with a specialty in Adult Health or Family Health.Responsibilities Develop a comprehensive health profile by collecting health and developmental history, analyzing clinical laboratory data and other diagnostic tests, and reviewing pertinent psychosocial data and health assessments. Perform complete or focused physical examinations, including measuring blood pressure, weight, and height. Order routine laboratory tests, such as CBC, blood chemistry, urinalysis, and serology, as necessary. Record findings from health exams on appropriate forms. Assess data, discriminating between normal and abnormal findings, and identify abnormalities that require further study. Assist in developing the medical section of the treatment care plan as a member of the interdisciplinary treatment team. Evaluate the treatment plan for effectiveness and make necessary adjustments or corrections. Make appropriate referrals to other community health care providers and specialists for consultation, tests, or treatments. Collaborate with other health disciplines to ensure continuity of care. Counsel clients and family members regarding the nature and course of illness, medication regimens, tests, or treatment procedures.
