How Youu2019ll Make an Impact as a Speech-Language Pathologist As a crucial member of our homeu2011based care team, you will: + Champion Elara Caringu2019s mission and philosophy + Assess and evaluate patient needs, establishing a personalized Plan of Care + Perform initial OASIS assessments and ongoing re-evaluations + Ensure patient safety through thoughtful care planning and resource coordination + Collaborate with physicians, supervisors, and interdisciplinary team members + Implement and revise Plans of Care based on patient progress + Provide direct speech-language pathology services according to clinical standards + Select therapeutic materials tailored to patient needs + Educate and support patients and caregivers on home therapy techniques + Maintain accurate, timely documentation and communication + Work in a dynamic, physically demanding environment with flexibility for irregular hours u2705 What Youu2019ll Need + Masteru2019s degree from an accredited program in Speech-Language Pathology + Current, unrestricted SLP license in Indiana + Minimum 1 year of clinical SLP experience + Ability to travel 50% within your service area + Reliable vehicle, valid driveru2019s license, and auto insurance + Ability to lift 50u2013100 lbs. and assist with patient transfers ?
