Within MSHP, Post-Acute Care Team is responsible for improving the quality of care our patients receive from post-acute care providers and reduce the overall cost of care by transitioning patients safely back to the community. MSHP seeks a Population Health Clinical Coordinator (PT/OT) to assess patient care in the Skilled Nursing Facility (SNF), monitor patient outcomes and the appropriate/timely discharges back to the community telephonically. This role is responsible for reviewing and evaluating Post-Acute Care services provided to MSHP population. The Population Health Clinical Coordinator (PT/OT) reports to the Post-Acute Care Manager. Qualifications Education and Experience ? Graduate of an accredited Physical Therapy/Occupational Therapy program ? Current New York State Physical Therapy/Occupational Therapy License ? Minimum 5 years of experience in skilled nursing facilities and/or acute care settings ? Experience in Utilization Management is a plus Additional Skills and Qualities ? Excellent relationship management skill and non-aggressive assertiveness ? Ability to problem solve, organized and high-level of attention to detail ? Computer literacy in EMR (Electronic Medical record) and Microsoft Suite (Word, Excel and PowerPoint) ? Effective communicator with excellent written and verbal communication to communicate with medical staff and management ? Demonstrated experience and record of consistently meeting and tracking timelines and objectives ? Fluency in English Non-Bargaining Unit, E01 - Partner MSO Services - MSH, Mount Sinai Hospital Responsibilities Responsible for reviewing and evaluating post-acute care services provided to MSHP population u2022 Identify on a daily basis, MSHP lives that are being serviced in SNFs u2022 Conduct patient reviews, assess the plan of care and reinforce the goal LOS u2022 Address and discuss potential discharge needs and barriers u2022 Assure SNF providers are implementing care pathways and best practice Responsible for reinforcing quality and evidence based care in an effort to reduce cost, optimize therapeutic outcomes and improve quality of life for MSHP patients u2022 Actively participates in clinical rounds, reviewing care plans with care providers u2022 Identifies progress toward desired outcomes and intervenes to overcome deviation in expected plan of care u2022 Generates quality improvement results u2022 Refers cases with quality care concerns to the Senior Director of the Care Continuum Responsible for identifying potentially unnecessary services and care delivery settings, and recommends alternatives if appropriate, by analyzing clinical protocols u2022 Functions as liaison between MSHP and the SNF u2022 Actively participates in discharge planning u2022 Focuses on inappropriate utilization of services while ensuring appropriate care is provided Responsible for the process of monitoring and evaluating the services and resources provided in response to a patientu2019s needs u2022 Oversees data collection process u2022 Reviews utilization patterns; identifies trends and problems About Us Strength through Unity and Inclusion The Mount Sinai Health System is committed to fostering an environment where everyone can contribute to excellence.
