Together, we are making a difference, one life at a time.Your Mission in ActionThe Clinic Medical Support Professional is responsible for providing South Plains area residents seeking non-emergency assistance with health screenings and basic health and wellness education, specializing in long-term diabetes care in accordance with Agency policies, licensing standards, and other federal, state, and local regulatory requirements.Provide patients with routine health screenings, such as, but not limited to blood pressure checks, blood sugar tests via finger stick, and temperature checksProvide patients with basic health and wellness education for patients with chronic health conditionsProvide health education to patients as it relates to promotion of proper health and wellness, nutrition disease preventionInitiate patient treatment plan based on prescribed health care needs and goals, physician orders, and health assessmentsMonitor and identify patient medical and behavioral changes and notify appropriate medical and/or behavioral health professional; document changes and responses using correct forms and documentation standardsAbility to drive to community partner sites and work with the publicComplete required documentation and record keeping based on licensing standards, Agency policies, and federal and state regulationsDocument and update patient charts/files with accurate and complete Work closely with Director and interdisciplinary team to provide the care that focuses on patients in needWork as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practicesAlways develop and maintain positive working relationships with patients and interdisciplinary teams within established boundariesWork as team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practicesMinimum QualificationsRegistered Nurse (RN) license in the state of Texas2 years’ direct patient care experienceValid Driver’s LicenseDemonstrated ability to use clinical skills in a fast-paced environment with competing requestsAbility to work under pressure using solid and appropriate decision-making skillsAbility to work collaboratively with diverse groups of peopleStrong time management and organizational skills with the ability to complete assigned tasks during shiftStrong interpersonal skills with the ability to effectively communicate with diverse peopleAbility to establish a positive rapport with clientsPreferred QualificationsBilingual in English and Spanish with the ability to fluently converse and write in Spanish (based on the population served)TravelThis position may be required to travel up to 25%Physical Demands & Work ConditionsThis position requires sitting and looking and using a computer for long periods of timePosition works within the communityPosition requires frequent standing and walkingLift, push, pull, move up to 25 lbs.
