Coordinates and collaborates with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfers. Coordinates and communicates capacity management plan to nursing units, leadership team and physicians. Possesses clinical knowledge and critical thinking skills to meet standards of care required by the patient population. Must have the ability to work in a fast-paced environment with a positive attitude towards all professional contacts.
