This role combines clinical insight, marketing, and coordination to ensure smooth transitions of care.Experience in healthcare marketing, admissions, or case management (preferred)Strong interpersonal and communication skillsAbility to work independently and manage a flexible schedule, including weekendsKnowledge of post-acute care services and insurance processes is a plusConduct on-site hospital visits to evaluate and engage prospective patients for admission to the facilityBuild and maintain strong relationships with hospital case managers, discharge planners, and clinical staffPromote the facility’s services and capabilities to increase referral volumeCoordinate and assist with admissions, including weekend coverageOversee and support patient sign-ins, ensuring accurate documentation and a seamless transition processCollaborate with internal teams to confirm appropriate placement and continuity of careTrack referral activity and provide regular updates to leadership
