Mental Health Counselor (Full Time/Tues-Fri 10a-6p, Sat 8a-4p) Buffalo, NY ( Apply Description This position is responsible for conducting mental health assessments, carrying a caseload, development of individual service plans, assisting in the development of a healthy mental state, and overall monitoring of residentsu2019 involvement in implementation of a sober support network. Additionally, this position assesses, diagnoses, and treats mental illness and psychological distresses within the context of the family system. Duties and Responsibilities u2022 Provides mental health screening, assessment, treatment planning, individual counseling, group treatment, and family intervention. u2022 Based on program need, will be responsible for carrying a caseload. Facilitates counseling sessions and groups with residents, couples, and family members. u2022 Works with residents to develop and reach treatment goals and revises goals as needed. u2022 Provides residents with realistic crisis and/or safety plans as needed. u2022 Conducts scales with residents to measure any increases or decreases in symptoms and adjusts interventions accordingly. u2022 Coordinates with psychiatrists as needed. u2022 Maintains up-to-date clinical documentation, progress notes, treatment plans, and discharge plans. u2022 Corresponds with relevant family members and partner agencies. u2022 Maintains knowledge of treatment modalities that are relevant to the resident population. u2022 Maintains individual case records in compliance with agency policy and licensing authority regulations. u2022 Coordinates services and maintains regular ongoing consultations with other organizations, managed care providers, legal agencies, etc. u2022 Collaborates with other clinical and medical staff members in advocating for residents during Medicaid managed care and insurance company reviews. u2022 Efficiently accesses other necessary services for residents as needed. u2022 Utilizes person-centered care and evidenced-based practices. u2022 Facilitates didactic/treatment phases and process groups. u2022 Supports and encourages resident participation in community self-help groups. u2022 Participates in supervision sessions with supervisor. u2022 Facilitates admissions and orientations of new residents as needed. u2022 Takes precautionary measures to limit transmission of communicable diseases (i.e., HIV, Hepatitis B, TB etc.). u2022 Serves on agency committees as requested. u2022 Carries out additional duties essential to the above-stated responsibilities as assigned. u2022 Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners. u2022 Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others. Requirements + Possesses a relevant license such as a LMSW, LCSW, LMHC, or LMHC-P. Masteru2019s Degree in Human Services or related field and a minimum of three yearsu2019 supervised counseling experience. Field placement as an intern may be applied to the required counseling experience. + In addition to the qualifications above, a valid New York State driveru2019s license is required. Salary Description $61,200/Year
