Team membersfrom all departments will focus on carrying out the EmberHope mission,vision, and valuesTo be a successful you must be able to do the following always:Ensure a comprehensive assessment of high-needs clients and assureappropriate referrals are made, services are accessed, and careproviders are supported.Conduct initial assessments by reviewing email correspondence, incidentreports, clinical assessments, complex case staffing, coordination ofcare staffing, Team Decision Making (TDM), and other referral sources toidentify high-acuity and exceptional clients.Review records, including mental health documents (e.g., acute, QRTP,and PRTF discharge summaries), medical records (e.g., birth records,psychological evaluations, school records), to identify gaps in serviceprovision.Collaborate with case teams to complete Universal Mental HealthReferrals and relevant documents (e.g., CAFAS, Child Behavior CheckList/CBCL) to be sent to appropriate Community Behavioral Health Clinics(CCBHCs) and Community Mental Health Centers (CMHCs).Collaborate with case teams to complete applications with SedgwickCounty Developmental Disability Organization (SCDDO) and acquiresupporting documentation for Intellectual and plete or provide guidance on referrals to access services.Collaborate with case teams to complete referrals for various waivers,including:1. Mental Health (SED waiver) Referrals: Complete Universal Mental Health Referrals and relevant documents for submission to CCBHCs and CMHCs.2. Intellectual/Developmental Disability (I/DD) Waiver Referrals: Complete applications with SCDDO and acquire supporting documentation.3. Technology Assisted (TA) Waiver Referrals: Collaborate with case teams and Managed Care Organizations (MCOs) to make referrals to the Children's Resource Connection.4. Brain Injury (BI) Waiver: Collaborate with case teams and MCOs to contact the Aging and Disability Resource Center (ADRC).5. Autism Waiver Referrals: Collaborate with case teams to submit online preliminary applications with the Kansas Department for Aging and Disability Services (KDADS), Autism Program Manager.6. Physical Disability (PD) Waiver: Collaborate with case teams and MCOs to contact the ADRC.Facilitate effective communication and coordination among Permanencyteams, internal and external providers to ensure seamless transitionsand appropriate service provision.Request, participate in, and facilitate meetings and staffings regardinghigh-needs children and youth in out-of-home care.Ensure timely and accurate exchange of client information with relevantentities, including Medicaid entities (MCOs), CCBHCs, CMHCs, privatemental health providers, community developmental disabilityorganizations, and other providers.Maintain detailed and up-to-date documentation of all communications.Ensure ongoing monitoring and support for clients after appropriatereferrals have been made to ensure service delivery and promotestability.Collaborate with case teams and placement providers to ensure intakeappointments are scheduled with plans for a knowledgeable adult andlegal guardian representative to attend.Conduct follow-up communication with case teams, caregivers, placementproviders, service providers, etc., to address any oncerns.Identify and monitor clients at high risk and provide additional supportand resources as needed.Collaborate with the clinical team to assess for higher levels ofcare/treatment (e.g., PRTF and QRTP), crisis supports (e.g., acutehospitalization, stabilization beds, mobile crisis support), andsubstance use treatment (inpatient and outpatient).Ensure quality services are in place for children and families and workto improve processes and procedures based on feedback and outcomes.Follow and adhere to Department for Children and Families (DCF) programoutcomes.Establish a feedback loop with clients, families, caregivers, andproviders to gather insights on the process.Foster a culture of interdisciplinary collaboration by holding regularmeetings with different departments, providers, and communityorganizations to discuss processes, concerns, and share best practices.Stay informed about best practices and emerging trends in carecoordination and incorporate them into practice.Collaborate with the training department to provide ongoing training andeducation for case teams on assessment, referrals, services, andsupports.Follow Company Policy and Procedure and operate according to theCompanys Program Practice G
