Kids In Crisis is Connecticut's ONLY free, round-the-clock agency providing emergency shelter, crisis counseling and community educational programs for children of all ages and families dealing with a wide range of crises, including domestic violence, mental health and family problems, substance abuse, economic difficulties and more.REPORTS TO: UC Lead Case ManagerRESPONSIBILITESCase Management:• Ensure all UC residents receive full range of assessments and program services• Maintain case management load of 4-8• Oversee safe and timely release services to ensure compliance with ORR policies and procedures• Accompany and transport UC residents to court and other appointments• Develop Individual Service Plans for each UC resident• Maintain confidential UC resident records / files.• Participate in weekly UC case review and Clinical meetings.• Meet weekly with UC Lead Case Manager to ensure success in program design and implementation• Transport/accompany clients to appointments and placements. Out of State travel via, agency vehicles, train or air.• Completion of agency trainings including but not limited to TCI, Med Administration, Phone Training, Crisis Intervention, Sexual Assault Crisis Training, ORR P&Ps, KIC P&Ps, UC program P&PsEssential Functions• Ability to lift a minimum of 20 lbs.; ability to Stand and sit for long periods• Other duties as assignedQUALIFICATIONS• English / Spanish fluency required• Bachelor's Degree in Counseling or the Human Services field required• Minimum 1-2 years post-graduate experience with clinical assessments and crisis intervention• Ability to work with multi-disciplinary team and knowledge of community resources a plus.• Excellent verbal and written skillsTo perform this job successfully, an individual must be able to perform each essential job duty satisfactorily.
