Provides care in accordance with HHA scope of service for providing personal care to includea. Bathing and hygieneb. Assisting with grooming and dressingc. Toileting2. Provides assistance with activities of daily living (ADL)3. Assists with ambulation and transfers4. Prepares meals following dietary instructions and assists with feeding as appropriate5. Assists client with shopping needs and errands6. Performs simple health tasks in accordance with a plan of care/service such as: reminding patients to self-administer prescribed medications, range of motion exercises, use of special equipment, and vital signs as instructed by RN7. Performs household and light housekeeping tasks such as tidying kitchen, dishwashing, linen changes, laundry, etc.8. Maintains a clean and safe environment within the client's immediate areas of activity9. Implements basic infection control procedures in accordance with agency policies10. Receives assignments and reports on tasks completed and need for changes11. Reports and documents changes in the patient's condition, any unusual occurrences, and accidents to the Nursing Supervisor12. Performs other supportive tasks in accordance with a care plan outlined by the Nursing Supervisor13. Completes timely, accurate, and complete documentation of care/service performed weekly in accordance with organizational policy14. Demonstrates understanding of the Client Bill of Rights and respect for the client’s privacy and property15. Complies with HIPAA regulations assuring confidentiality in all aspects of client, employee, and agency information.16. Communicates clearly and effectively with clients, coworkers, and supervisory staff17. Submits annual health screening, credentialing, and education update documentation required for HR record prior to expiration dates18. Completes required in-service training and competency evaluations19. Participates in Performance Improvement activities
