Home care offers valuable real-world perspective on what truly helps patients restore their health.WHAT WE OFFERWe make it easy to do your job and provide competitive financial incentives, including:Guaranteed base salary plus a generous uncapped bonus structureMileage reimbursement and additional bonus opportunitiesFlexible schedulesFull team support (scheduling, patient admissions, etc.)Our benefits package includes:Medical, dental, and vision insurance with flexible optionsPaid time off401(k) participationCompany-paid life insuranceEmployee Assistance Program (EAP)HOW YOU'LL WORKProvide comprehensive skilled services as ordered by the attending physicianEducate patients and their family members while ensuring patient safetyComplete accurate documentation and remain compliant with all legal and regulatory requirementsQUALIFICATIONS & ATTRIBUTESCurrent Physical Therapist license in the state of employmentMinimum of one year of experience as a PT in an acute setting (required)Home care experience preferredStrong communication and interpersonal skillsKnowledge of therapy best practicesAbility to make sound clinical judgmentsProblem-solving skills and adaptabilityDetail-oriented and observantStrong work ethic with the ability to prioritize and be timelyValid driver’s license and auto insurance (must be listed driver)MAJOR AREAS OF RESPONSIBILITYProvide skilled physical therapy in patients’ homes as ordered by the attending physicianObserve, record, and report patient reactions, changes, and progressPerform OASIS assessments and adjust plans of care as neededComplete initial evaluations and admissions within 48-72 hours of referralDevelop and manage individualized plans of careHelp decrease re-hospitalizations, especially for high-risk patientsTreat patients to relieve pain and restore function using physical methods (exercise, massage, heat, water, etc.)Arrange outpatient services when care cannot be provided in-homePromote continuity of care (admissions, transfers, discharges)Educate and counsel patients and families on conditions and care plansInvolve patients and families in plan of care developmentInstruct patients on proper use of assistive devices (wheelchairs, walkers, crutches, etc.)Ensure patient/family understanding of financial responsibility for equipmentSupervise PTAs and, when appropriate, nurse assistantsEvaluate patient response and adjust plan of care accordinglyCommunicate with PTAs and nurse aides regarding supervisory visitsComplete all documentation per agency and Medicare/Federal guidelinesParticipate in audits, meetings, and training sessionsMaintain active licensure and professional standardsESSENTIAL FUNCTIONSTravel to patient locations as scheduledMaintain daily attendance at assigned visit locationsComplete documentation within required timeframes:Within 24 hours for routine visitsWithin 48 hours for start-of-care visitsSubmit all required documentation within company standardsEstablish and follow plans of care within scope of practiceHEALTH QUALIFICATIONSHealth RequirementsMust meet all physical job requirementsProvide proof of a current negative TB test (or complete testing at orientation)Annual TB testing required unless prior documentation existsMust be free of conditions that would limit job performanceMay be exposed to bloodborne pathogensPhysical DemandsBelow are the minimum physical requirements for this position.
