Job Title: Public Health Nurse II (PHN) + Location: Department of Family Services u2013 Adult & Aging Division (Virginia) + Job Type: Full-Time / Contract Job Description: Join a mission-driven public health team within the Adult & Aging Division of the Department of Family Services, where your nursing expertise supports vulnerable populations across the community. As a Public Health Nurse II, you will play a critical role in assessing eligibility for Medicaid-funded Long-Term Services and Supports (LTSS) and delivering essential public health nursing services. This role offers meaningful, community-based work focused on advocacy, safety, and the well-being of older adults, individuals with disabilities, and families. Key Responsibilities: + Serve as a certified Long-Term Services and Supports (LTSS) Screener on a multidisciplinary team + Conduct home and field visits to assess client health, safety, functional needs, and level of care in accordance with state and federal (DMAS) guidelines + Perform comprehensive assessments using Virginiau2019s Uniform Assessment Instrument (UAI) + Develop, coordinate, and monitor person-centered care plans + Provide case management, care coordination, crisis intervention, and appropriate referrals + Implement public health nursing services including outreach, case finding, health promotion, education, and consultation + Work with diverse and at-risk populations, including older adults, adults with disabilities, medically fragile individuals, and children + Maintain accurate documentation and ensure compliance with applicable laws, policies, and professional nursing standards + Collaborate effectively with internal teams, community partners, and interdisciplinary team (IDT) members + Participate in required meetings, trainings, and continuing education to maintain core nursing competencies Qualifications: + Current Registered Nurse (RN) license in the state of Virginia + Experience in public health, case management, community health, or LTSS strongly preferred + Ability to obtain and maintain LTSS Screener certification + Strong assessment, care planning, and care coordination skills + Demonstrated ability to manage a caseload and prioritize competing demands + Excellent written and verbal communication skills + Ability to work independently and collaboratively in diverse community settings + Valid driveru2019s license and ability to travel locally for home and field visits + Must meet all federal, state, and local employment requirements + Must be at least 18 years of age Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: + Competitive pay & weekly paychecks + Health, dental, vision, and life insurance + 401(k) savings plan + Awards and recognition programs Benefit eligibility is dependent on employment status.
