Predictable Schedule: Enjoy a stable, outpatient-focused schedule that supports work-life balance Team-Based Care Model: Work alongside physicians, nurses, medical assistants, behavioral health professionals, and care coordinators Supportive Leadership: Accessible leadership team focused on provider success and professional growth Loan Repayment Eligibility: Participation in the National Health Service Corps (NHSC) / HRSA Loan Repayment Program Mission-Driven Work: Make a meaningful impact by serving an underserved and diverse Brooklyn community Modern Clinical Environment: EMR-supported workflows and resources designed to enhance efficiency and patient care Key Responsibilities Provide comprehensive primary care services across the lifespan (pediatrics, adults, and geriatrics) Perform thorough patient assessments, including medical histories, physical exams, and preventive screenings Diagnose and manage acute and chronic conditions such as diabetes, hypertension, asthma, and other common illnesses Develop and implement individualized treatment plans in collaboration with patients and care teams Prescribe medications and therapies in accordance with New York State regulations Deliver patient education focused on disease prevention, health maintenance, and lifestyle modifications Maintain accurate, timely, and compliant electronic medical records (EMR) documentation Coordinate care with specialists, behavioral health providers, and community resources Participate in quality improvement initiatives and adhere to FQHC clinical standards and guidelines Qualifications Active and unrestricted (LPN) Nurse Practitioner license in the State of New York DEA license (or eligibility to obtain) Experience in Family Medicine, Primary Care, or Community Health strongly preferred Prior FQHC or underserved population experience is a plus Strong clinical judgment and ability to work independently within a collaborative team Bilingual (Spanish or other languages) is a plus but not required Ideal Candidate Profile Passionate about community-based healthcare and health equity Comfortable managing a diverse patient population with varying levels of medical complexity Strong interpersonal skills with a patient-first mindset Adaptable, team-oriented, and mission-driven Work Environment This is a fully outpatient role within a well-established FQHC setting.
