Degree in Physical Therapy, active Physical Therapist license and 1+ year of Physical Therapy experience required. Applicants who do not meet these qualifications will not be considered. Deliver comprehensive physical therapy services in this role focused on restoring function, enhancing mobility, and improving patient independence across inpatient and outpatient settings. This contract opportunity involves working closely with the rehabilitation team and physicians to assess, plan, and implement personalized treatment programs tailored to a diverse patient population. Key Responsibilities: Evaluate patients using physical therapy techniques and modalities such as Ultrasound, NMES, and manual muscle testing. Develop and adjust individualized treatment plans aligned with physician orders and patient goals. Supervise Physical Therapy Assistants and collaborate with rehab team members to achieve optimal clinical outcomes. Maintain accurate, timely documentation in compliance with regulatory standards, including evaluations, progress notes, and discharge summaries. Monitor treatment efficacy, recommending modifications as needed. Ensure patient safety and abide by all departmental safety protocols. Participate in departmental budgeting needs and performance improvement initiatives. Desired Qualifications: Current Illinois Physical Therapist license required. Active professional licensure and at least 1 year of physical therapy experience preferred. Valid driver’s license with proof of auto insurance. BLS certification required; APTA membership is a plus. Proficient in Microsoft Word, Excel, Outlook; computer literacy essential. Strong organizational skills, independent work capability, and excellent communication.
