Job Title: RN Case ManagerJob Description The RN Case Manager provides comprehensive care coordination across the continuum in an acute, Level I trauma, tertiary academic medical center. This role conducts initial assessments for patients in observation or inpatient levels of care, develops safe and viable discharge plans in collaboration with patients, families, and caregivers, and partners with a multidisciplinary team to ensure appropriate resource utilization. The RN Case Manager collaborates closely with utilization review nurses and case management assistants to support authorization processes and post-acute referrals, while maintaining a strong focus on patient-centered care and clinical excellence. Responsibilities + Conduct thorough initial case management assessments for patients in observation or inpatient levels of care to identify discharge planning and care coordination needs. + Develop individualized, safe, and viable discharge plans in collaboration with patients, families, and designated caregivers. + Coordinate care across the continuum by working closely with the multidisciplinary team, including physicians, nurses, therapists, social workers, and support staff. + Participate actively in multidisciplinary rounds to communicate patient needs, discharge barriers, and care coordination plans. + Meet with patients and family members or other designated participants in care to discuss discharge options, post-acute services, and available resources. + Assist with obtaining authorization for post-acute care services, including skilled nursing, home health, rehabilitation, and other community-based resources. + Collaborate with utilization review nurses to monitor authorization of care and ensure appropriate level of care and medical necessity from admission through discharge. + Perform utilization reviews when needed or assigned, applying established criteria and standards to support medical necessity and appropriate resource use. + Apply the full nursing processu2014assessment, care planning, intervention, and evaluationu2014within a patient-centered professional practice model as it relates to case management functions. + Maintain a solid working knowledge of specialized case management and utilization management methodologies and practices, and apply these concepts to daily work. + Ensure that medical appropriateness criteria and medical necessity for level of care are met, clearly documented, and updated throughout the hospitalization. + Develop and oversee implementation of an interdisciplinary plan of care that integrates clinical needs and discharge planning, ensuring it comprehensively meets the patientu2019s ongoing healthcare needs. + Reevaluate the plan of care regularly and adjust discharge plans based on changes in the patientu2019s condition, needs, and available support systems. + Work efficiently and effectively in a fast-paced acute care environment, prioritizing tasks and managing multiple cases simultaneously. + Exhibit a positive mindset and adaptability in response to changes in workforce, clinical practice, and operational settings. + Engage in opportunities to influence decision-making related to bedside clinical practice and care coordination processes. + Work calmly, maintain professionalism, and respond courteously under pressure, particularly in a high-acuity trauma and tertiary care environment. + Collaborate with case management assistants on the post-acute referral process to ensure timely and appropriate placement and services. + Uphold organizational values by contributing to compassionate, competent, and comprehensive healthcare delivery, and supporting a culture of learning and development. + Work every other weekend as part of the regular schedule to support continuous case management coverage. Essential Skills + Active and unencumbered California RN license. + Bacheloru2019s degree in nursing. + Active Basic Life Support (BLS) certification provided by the American Heart Association. + Minimum of 2 years of inpatient acute care experience in a hospital setting (non-ED, non-clinic). + Minimum of 2 years of acute inpatient experience, preferably in a Level I trauma hospital. + Minimum of 2 years of hospital case management, care coordination, or utilization management experience. + Experience in discharge planning within an acute care hospital environment. + Demonstrated ability to perform comprehensive initial assessments, care coordination, and development of viable discharge plans with patients and families. + Strong working knowledge of case management and utilization management practices and methodologies. + Knowledge of state and federal healthcare regulations and accreditation requirements related to case and utilization management. + Ability to work efficiently in a fast-paced acute care setting and manage multiple priorities. + Strong interpersonal and communication skills for effective collaboration with patients, families, and multidisciplinary teams. + Ability to exercise clinical expertise, discretion, and independent judgment in applying the nursing process to case management functions. + Ability to work every other weekend as part of the regular schedule. Additional Skills & Qualifications + Experience working in a Level I trauma facility. + Experience in a large tertiary academic medical center. + Nationally recognized certification in Utilization Management or Case Management (preferred). + Familiarity with patient-centered professional practice models such as Duffyu2019s Quality-Caring Model. + Demonstrated ability to adapt positively to changes in clinical practice, operations, and workforce dynamics. + Strong problem-solving skills and the ability to address complex discharge barriers. + Commitment to continual learning, professional development, and participation in a culture that values education and growth. + Ability to engage in and support initiatives that improve bedside clinical practice and care coordination processes. Work Environment This position is based in a large, approximately 400-bed, Level I trauma, acute care tertiary academic medical center that emphasizes continual learning, professional development, and career growth within a robust case management team. The RN Case Manager works day shift hours, typically from 8:30 a.m. to 5:00 p.m., and is scheduled to work every other weekend to support continuous coverage. The environment is fast-paced and clinically complex, with a strong multidisciplinary approach to patient care and a focus on comprehensive, compassionate, and competent healthcare.
