Employment Type: Full time Shift: Day Shift Description: Job Summary Provides centralized scheduling of Registered Nurse Navigators for Medicare AWV (Annual Wellness Visit) appointments. Assists to develop a schedule which facilitates the most efficient use of each nurseu2019s schedule. Manages clinical information and conducts data abstraction to identify patients with gaps in care. Conducts outreach to patients who need to complete Medicare AWV. Is an integral member of the health care team and works to ensure safety, best practice, and high quality standards of care are maintained across the continuum. Job Duties + Proficient in computer and internet skills to access payersu2019 websites. May perform manual eligibility on Medicare Website. + Conducts data abstraction from various electronic systems. Follows clinical procedure/protocol for the organization and management of the electronic medical record (i.e. test results, retrieval, filing, creation repair of electronic medical record). + Provides proactive outreach phone calls to patients due for Medicare AWV. + Schedules appointments for multiple nurses and assists in maintaining organization of schedules. May schedule tests, procedures, and referral appointments as trained and communicate with patients regarding these appointments. + Answer phone calls for multiple nurses, transfer calls to nurse or create task within electronic medical records according to protocols. Hours Full Time; 36 hour/week Minimum Qualifications + CMA or LPN, or similar healthcare certification required + Background of at least 1-year medical experience required. Experience within a clinic/physician office preferred + Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks). + Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
