Job Summary: Provide supportive services to residents of McDonough County experiencing financial crisis. Use the Pathway of Hope strengths-based case management approach to individuals and families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives. Responsibilities Essential Responsibilities: + Maintain a positive work environment where at-risk and disadvantaged individuals can come to address their needs + Assist clients expeditiously with courtesy, understanding and empathy + Receipt and document donations received + Coordinates all aspects of the Food Pantry including but not limited to in-kind donations, ordering/purchasing food and supplies, box preparation and distribution + Attend staff meetings and community events as needed + Maintain an organized, professional work environment + Develops and fosters local relationships with other service/ healthcare/ educational/ municipal entities to ensure effective local support and referral sources. + Act as a liaison and/or coordinator between agency/business resources as it relates to financial or other client need + Responsible for administration of: + Pathway of Hope Program/Supportive Housing Program + Warm Neighbors Cool Friends + Comprehensive Emergency Assistance Program + Homeless Prevention Program + Provides referrals for participant to needed services through other providers as needed + Conduct client intakes and assess the level of need; completing all appropriate assessments and paperwork, gathering documentation per the requirements laid out by IDHS program guidelines. + Determine client eligibility for various programs/referral systems and provide assistance within program parameters + Schedules regular meetings with clients to set goals and track progress + Establishes client driven service plan and updates the plan regularly, at a minimum of on a quarterly basis + Maintains accurate client case files, including intake, service plan, identity documentation, case notes, phone calls, interviews, discharge, and follow-up + Investigates and documents grievances, incident reports and violations of participant’s rights + Maintain and submit monthly statistics as required for The Salvation Army and other grants received + Monitors expenses and balances of all funds used to provide emergency financial assistance + Participate in renewal of all contract and funding plan requests + Coordinates and implements various special programs such as annual coat collection/distribution + Initiates and coordinates the application process for the Christmas Assistance programs including Toys-For-Tots® and food baskets, and coordinates toy and food distribution + Supervises, trains, and provides guidance and support to mentors, student and community volunteers. + Collects, compiles and reports statistical data to the corps officer and Pathway of Hope Regional Coordinator + Promote an "all hands on deck" approach to teamwork in the office; assisting where needed + In collaboration with the corps officer, develop and maintain policies and procedures for the Emergency Assistance and Pathway of Hope programs + “Safe from Harm” Certification. (required) + Salvation Army Case Management Certification (required) + Perform other duties as assigned as related to overall Social Ministries outreach Position Ranking: + Regular, Part Time, Non-Exempt Qualifications Qualifications: Candidates must have a passion for fulfilling the mission of The Salvation Army and must be articulate in presenting the same to others, possessing ability to relate with warmth, understanding, acceptance and empathy. Candidates must successfully pass the pre-employment background inquiry and annual motor vehicle check with a valid driver’s license. Education: Bachelor's degree preferred, significant related experience may be considered in lieu of a degree. Skills: + Ability to efficiently and effectively manage people and projects successfully. + Team player, self-starter, and perform well with minimal supervision. + Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner. + Ability to establish policies and procedures; holding others accountable to the same. + Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member. + Demonstrated awareness of budget resources and cost control, as well as planning and administering budgets related to social ministries. + Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, Internet and Social Media platforms. + Must be able to work days, evenings, weekends and holidays as necessary. Especially in emergent situations. Physical Demands/ Work Environment: This position is required to do basic business and office work. In order to successfully perform the essential functions of this position, the individual is regularly required to talk and listen, stand, walk, sit and use hands. Individual must be able to regularly lift up to 30 pounds and occassionally lift up to fifty pounds. The use of Office and Computer Equipment is necessary. Vision abilities will require close vision. Work may be performed indoors and occasionally require outdoors to attend meetings. There is low to moderate noise level for this position.
