Duties and Responsibilities:Responsible for review and follow up of delinquent patient accountsMonitors the overdue balance report on a daily basisMeets with patients on a weekly basis (or more if necessary)Preparation of financial agreementsDocumentation of interventions in preparation for financial dischargesWeekly meeting with ED and DOF to review noncomplianceSubmits MA applications online via COMPASS websiteFor existing patients who lose their MA coverage,For existing patients who would like to switch from self-pay to MA coverage,For prospective patients who need assistance with applying.Follows up on MA applications:Weekly meetings with patientsFollow up via phone and email with DHS caseworkers as necessaryMust have a deep knowledge of necessary forms, eligibility criteria, and application process (I will train on this)Monitors current patient Medicaid eligibility via PROMISe websiteAdjusts patient account information when coverage changes (activates or deactivates)Screens potential admissions for Medicaid eligibilityCoordinates with other insurances or funding sources as necessary (Veterans Administration, Drug and Alcohol Commission, etc.Coordination of care between MH, PH, SUDGather history and care status from external providers and stakeholdersConnection to community resources surrounding employment, DV, housing, transportation, government assistance, legal, child care, support services (groups, support network, respite)Serve as communication coordinator between internal staff and external providersDevelop a case management plan with the individual where one goal is to no longer need case management in most situationsIncomplete the Virginia IPOC every 30 days as long as the person is involved in CMProvide updates on active CM patients during treatment team.Obtain consents for all referral sources and existing supports which will include diagnosis and treatment plan for health providers when possibleDocumentation of in-person meetings, phone calls, referrals, support, planning and follow upWork with primary counselor to ensure primary treatment plan is updated to include case management services with the reason(s) for the referralOther duties as assigned. Qualifications:Bachelor’s degree.Needs to be highly organizedDetail orientedExtremely approachable and customer service orientedAbility to develop contacts and network (good communication skills)Healthcare account management or case management experience would be a plusMust be available to work at least 1 weekday starting at 4:30am to provide an opportunity to meet with early dosers.
