This candidate will work 9 hour shifts Monday-Thursday and will work a 4 hour shift on Friday. The Nurse LPN Outpatient is part of the person-centered integrated care team and provides office-based specialized services to primary health care patients and assist in managing the patients full array of physical health care needs, in addition to behavioral health care needs through a "whole-person" approach. This Nurse is responsible for the operation of the Medication Clinic. Functions as a case manager for consumers served by medication clinic. Plan, organize and coordinate medication clinicPrepares for minimal to frequent schedule changes and travels to neighboring clinics for nursing coverage and trainingReview medication education sheets with clientFunction as case manager for consumers needing medication refillsComplete necessary documentation regarding medication clinic and psychiatric consultation for consumer recordsAdminister IM medication, dispense sample medications, maintain accurate logs on all medications received and dispensedWork to help complete prior authorizationsMaintain list of injections, coordinate ordering and delivery, verify orders are current, and discuss any side effectsTake vital signs and coordinate prediabetes screening processBroad knowledge and experience with computersOther duties as assignedHigh School/GED requiredPractical Nursing certificate requiredLICENSURE/CERTIFICATION: LPN or GPN that must pass the MO NCLEX exam within 90 days of graduation requiredCompass Health Network is a nonprofit health care organization offering accessible, comprehensive, & compassionate behavioral health, substance use treatment, family medicine and dental services throughout Missouri.
