Shift: 8:00 AM - 5:00 PM Experience: • 12 months of role experience is required. • 24 months of Home Health experience is required. • Experience with Epic is required. • Experience with Oasis is required. Requirements: • Candidates must have a Maine license or compact license in hand at the time of application. • This role may require floating to additional units and locations • Candidates must live at least 90 miles away from this facility in order to be considered a traveler when applying for this role. • COVID vaccination required for onboarding. Religious and medical declinations accepted. • Flu vaccination required for onboarding. Medical declinations accepted. Pre-employment modules may be required for this role. Additional Details: Weekly Guaranteed Hours: 40 Weekend Hours: 36 (unless otherwise noted): Every other weekend rotation with a day off during the week. Or a permanent schedule of Sunday-Thursday w/ Friday and Saturday off- Based on facility needs at time of Contract Call Requirements No overnight call. Call back required 2-4 times a month between the hours of 1700-2000. You will be told by 1700 on that day if an extra visit is needed. If by 1700 there is no visit scheduled, you are no longer on call. Patient Ratios: Case Manage 20-25 patient with an expectation of 5-6 visits per day; may use as re-vistor if agency needs change at any time during contract Must have Home Health experience.
