Providing care to a diverse and underserved population in an outpatient pany InformationFederally Qualified Health Center that provides primary and specialty healthcare.Job Duties Performs Mental Health Assessments for patients to ascertain their diagnoses and establish treatment plans Provides individual and group counseling and psychotherapy Provides crisis intervention andor counseling to diffuse conflicts among patientsCompletes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinicsMakes andor tracks all referrals to other mental health agencies and providersWorks collaboratively with specialists for consultation and referral purposesWorks with others on the intake team to help determine whether or not a patient would be best helped by the facility or another agency.Meets with other clinical staff to review casesMaintains confidential case files, including documenting requests for confidential informationFollows the clinical chart review policies and proceduresWorks in a team environment Provides workshops for families on communication and other family-related issues Coordinates emergency responses, including a team to minimize patient's danger to self and others, direct discussion with EMSFDNY regarding patient's condition, referral to psychiatric or medical emergency departments and follow-up while patient is in the emergency department or upon releaseMaintains ultimate responsibility for directing the course of the patient's mental health care and treatment plan within the scope of hisher expertiseEncourages health promotion, disease prevention, and health maintenanceProvides patient advocacy, as appropriate for the siteParticipates in case-conference and case management activitiesEducationUnrestricted New York State Clinical Social Worker license. Experience & Skills RequiredMinimum of two years' clinical experience preferable.Familiarity with word processing and other computer applications.Demographic, Registration, AppointmentScheduling, Scanning, Triage, PMHVitals, Orders, ImmunizationsPPD, Progress Note, Reporting, Labs, Medical Summary, Telephone Encounters.
